Responsibilities:

This position is responsible for developing and implementing the health plans housing strategy for LTSS programs. This includes the approach to assisting individuals in LTSS programs in securing accessible, affordable housing through Federal and local programs

Key Accountabilities and Responsibilities

  • Developing approaches to assist individuals in LTSS programs in securing accessible, affordable housing through Federal, state and local programs. This includes, but is not limited to, collaborating with federal, state and local affordable housing agencies such as Housing of Urban Development (HUD), State Housing Authorities, subsidized housing, public housing authorities and voucher programs

  • Establishing innovative partnerships with such housing agencies to develop and increase access to affordable and accessible housing options, particularly for those who wish to transition to the community.

  • Partner with lead trainers to develop and implement training specific to affordable housing services.

  • Support Case Managers in identifying housing options for LTSS members.

  • Serve as subject matter expert and organization resource in housing for people served through Long-Term Services and Supports (LTSS) programs.

  • Develop standards and procedures to be used in supporting the NF and MCO transition staff in development and implementation of housing resources.

  • Representing AmeriHealth Caritas in community meetings and workgroups relevant to housing issues of our members and will help secure housing for those members experiencing housing insecurity and homelessness

Education/Experience:

  • Bachelor’s Degree

  • Three to five years of experience in developing housing and/or housing policy at a local, State or federal level for special populations such as the elderly, people with disabilities and/or people with mental health challenges to secure accessible, affordable housing through Federal and local programs

  • Ability to leverage knowledge and experience of affordable housing programs for identified populations at a local, State and federal level to identify and develop housing opportunities.
