The Behavioral Health Coordinator is responsible for acting as a liaison and support person between the Community Health Choices Service and Care Coordination teams, the BH Managed Care (BHMCO) and Behavioral Health providers. The Behavioral Health Coordinator will be responsible for outreach activities with participants and providers and will be responsible for documenting all activity in electronic case management documentation system.
The Behavioral Health Coordinator is responsible for:
Coordinating Participant care needs with BH Providers.
Developing processes to coordinate behavioral healthcare between Primary Care Physicians and Behavioral Health Providers.
Participating in the identification of best practices for BH in a primary care setting.
Coordinating behavioral care with medically necessary services.
Being knowledgeable of the BH Managed Care Agreement requirements and coordinating with other BH-MCOs to effectuate the requirements.
Three to five years prior experience in behavioral health/human services required; or other equivalent background and experience that would translate well to this position.
Medicaid Managed Care Experience strongly preferred.
Proven liaison, customer engagement, independent project management experience required.
Proven ability to interpret policies and manage compliance.
